Cross Cultural Care Center (CCCC) is a flexible and responsive to the needs of our client, we offer quality services at reasonable cost/ fee.

Our good/ quality services not only serves our clients interest but also that of the working members of their immediate family which comes as respite, those home care-givers who work full or half time will no longer worry about their loved ones while at work. Their loved ones will enjoy the comfort of our services in a serene and friendly atmosphere. Our clients can also access half day or full day care services depending on their preference/ choice.

Our day care center offers programs designed for persons;

Who are physical and cognitively impaired and unable to perform their activities of daily living independently
With chronic health problems who require regular health and medical supervision.
Newly discharged from an acute care hospital or rehabilitation center.
Who need social stimulation, spiritual and emotional support.
Who are in need of specialized programs due to memory or cognitive losses
Older persons with disabilities, physical, developmental and cognitive impairments.
Younger adults with physical disabilities can also benefit from our programs.

Cross Cultural Care Center (CCCC) is a newly established Adult Day Care center that offers day care services to young adults and older persons from different cultural background and faith. Our Day Care Center therefore offers integrated care services to the older people in a serene, peaceful and conducive environment. The Center’s Day Care program is designed to meet the needs of older adults with functional impairments through an individual plan of care. It is a structured, comprehensive program that provides a variety of health, social and related support services in a protective setting during any part of a day.

Most people associate long-term care with the elderly, but it applies to the ongoing care of individuals of all ages who can no longer independently perform basic activities of daily living such as bathing, dressing, or eating-due to all illness, injury, or cognitive disorder. Loss of strength or ability may mean that people can’t get to places or do things they used to enjoy, they may be less able to communicate in social situations. The purpose of our Center is to help the older people with above challenges to cope with the demands of their daily lives and give them the comfort to continue living with dignity and happiness.

Our goal is therefore to provide well-being through the provision of quality social and health care related activities. We understand the importance of Adult Day Care Health services means for impaired adults and older people and their family care givers, the satisfaction is well worth the time, money and efforts.

The center provides services on a flexible schedule so that many different needs for caregiver relief can be accommodated. Some participants attend daily or several days a week to allow employment opportunities for primary caregivers; others attend for (a few hours) (occasional) full days so that caregivers can attend activities of their choice during the day. Need assessment and Best interest determination (BID) will be conducted during admission that will determine which care to be given and routine management plan for the client.
